USER CONSENT TO PERSONAL DATA PROCESSING

(in accordance with Federal Law No. 152-FZ of July 27, 2006)

Any individual who is a user of the website https//italyvms.com, when making an appointment to submit documents by clicking the "Next" button on the "Check the accuracy of the data" page, decides to provide their personal data to Visa Management Service LLC, OGRN 1087746305624, located at: Russian Federation, 119017, Moscow, Maly Tolmachevsky Lane, 6, building 1 (hereinafter referred to as the "Personal Data Operator"), and consents to their processing voluntarily, of their own free will, and in their own interests, under the following conditions:

1. The purpose of processing personal and biometric data is to fulfill of individuals’ (personal data subjects) orders, based on the Services Agreement concluded in simple written form for with the Personal Data Operator for the provision of services for the acceptance and transmission of documents to the Consulate General of Italy in Moscow or the Consulate General of Italy in St. Petersburg for the purpose of obtaining entry visas for tourist and other trips to Italy, as well as the acceptance of documents from the Consulate after complete of all formalities and their transfer to an individual.
2. List of personal data for the processing of which the personal data subject consents: last name/last name at birth, first name, patronymic; date of birth; place of birth; country of birth; citizenship/citizenship at birth; gender; marital status; for minors: last name, first name, patronymic, address and citizenship of guardian/legal representative; home address; registration details; telephone number; email address; passport details: series, number, issuer, date of issue; internal passport details; birth certificate details; details of the document cofirming legal residence in the Russian Federation; profession; income; name and address of place of work/educational institution; work telephone number; first and last name of the inviting person; name of the inviting company/organization/educational institution; name of the hotel; address of the inviting party; telephone number, fax number, email address of the inviting party; biometric data: photograph, fingerprints.
3. Consent is given for the following actions with personal and biometric data: The processing of the above personal data will be carried out through mixed processing of personal data (collection, systematization, accumulation, storage, clarification (updating, changes in use, distribution), transfer (including cross-border transfer), depersonalization, blocking, and destruction of personal data. A general description of the above data processing methods is provided in Federal Law No. 152 of July 27, 2006.
4. The period when the consent of the personal data subject is valid, as well as the method of its revocation, unless otherwise provided by federal law: the personal data of the subject will be stored for the periods established by law. Personal data shall be destroyed upon achievement of the purposes of personal data processing; in the event of liquidation or reorganization of Visa Management Service LLC; based on a written request from the personal data subject with a request to stop processing his personal data (the operator will stop processing such personal data within 3 (three) business days, of which a written notice will be sent to the personal data subject within 10 (ten) business days).

Upon written request, the subject has the right to receive information regarding the processing of their personal data (in accordance with paragraph 4 of Article 14 of Federal Law No. 152 of June 27, 2006).

I confirm that, in accordance with paragraph 1 of Part 4 of Article 18 of Federal Law No. 152-FZ of July 27, 2006, "On Personal Data," my family members and close relatives, as well as persons inviting me to a foreign country, will be notified by me of the processing of their personal data listed below: last name, first name, patronymic; date of birth; information on family relationships; contact telephone number; contact email address; actual address of actual residence or place of stay; information on current citizenship; name, series, and number of a valid identity document.

I confirm that I have got acquainted with the provisions of Federal Law No. 27.07.2006 No. 152-FZ "On personal data", the rights and obligations in the field of personal data protection have been explained to me.
