- Passport or travel document valid for at least three months after visa expiry date.
- Return-trip booking or ticket or proof of available means of personal transport. If travelling by car the following documents are required:
- original and a copy of the Vehicle Registration Certificate;
- original and a copy of the International Insurance Policy with civil responsibility (“green card”);
- original and a copy of driving license;
- original and a copy of the car rent voucher if the car is rented. If an applicant is going to visit other Schengen countries the applicant should submit a complete itinerary of the trip;
- if the driver with whom the trip is planned has a valid Schengen visa, to submit documents the applicant should provide a hotel reservation with the driver’s name and surname, a copy of the passport page with a photo and personal data, a copy of the valid Schengen visa.
If you plan to visit several countries in Schengen Area, you should provide a whole route of your trip.
- Medical/health documentation comprised of:
- medical documentation issued by the country of residence attesting to applicant’s illness;
- declaration by a public or private Italian health care facility (private clinic must be accredited by the national health service) indicating type of treatment, date initiated, estimated duration and costs;
- document from the Italian healthcare facility confirming the deposit of at least 30% of the estimated costs of the treatment requested or, alternatively, a specific administrative act passed by an Italian Region or specific authorization issued by the Ministry of Health within the framework of humanitarian programs.
- Documentation proving the availability in Italy of resources sufficient for payment of remaining healthcare expenses, food and lodging outside the healthcare facility, and repatriation of the patient and eventual companion.
- For the patient’s eventual companion: minimum health insurance coverage of €30,000 to cover repatriation and medical treatment and/or emergency hospitalization expenses.
