Minors
To submit documents for minors, in addition to the documents required should be provided:
• a minor’s birth certificate (in original and a copy / a verified copy by notary). If a parent or minor have changed surname after receiving the child’s birth certificate, confirming documents should be provided (i.e. marriage certificate, divorce certificate, certificate of name change (in original and a copy / a verified copy by notary);
• a filled application form signed by both parents (the last point on the last page of the application form) with a minor’s photo (photo requirements);
• if a minor is travelling with parents:
- a confirmation of an accommodation for the trip shared with the minor;
- round-trip tickets for parents and minor;
- copies of parents’ valid visas, foreign passport page containing personal data and their passports in original (in case of visa is applied for a minor only and parents have already valid Schengen visas).
• if a minor is travelling with one of his parents:
- a confirmation of an accommodation for the trip shared with the minor;
- round-trip tickets for a parent and a child;
- a copy of parent’s valid visa, foreign passport page containing personal data and his passport in original (in case of visa is applied for a minor only and parent has already a valid Schengen visa);
- a notarized consent form, issued no more than one year ago, signed by a parent who doesn’t accompany a minor within a trip (both original and a copy / a verified copy by notary);
- copies of a national passport of the parent who signs a notarized consent form (pages containing personal data and registration stamp).
• if a parent leaves the Shengen countries earlier/later than his child, or arrives there earlier/later, or leaves the Shengen countries for some time during one trip:
- a notarized consent form, issued no more than one year ago, signed by this parent (an original document and a copy / a verified copy by notary).
- copies of a national passport of the parent who signs a notarized consent form (pages containing personal data and registration stamp).
• when applying for a long visa for a minor:
-a notarized consent form, issued no more than one year ago, confirming that both parents allow each other to accompany a minor;
-copies of a national passport of the parent who signs a notarized consent form (pages containing personal data and registration stamp)
• if a child travels with an attendant:
- a confirmation of an accommodation for the trip shared with a minor;
- round-trip tickets for the attendant and a minor;
- a copy of attendent’s valid visa, foreign passport page containing personal data and his passport in original (in case of visa is applied for a minor only and attendent has already a valid Schengen visa);
- a notarized consent form, issued no more than one year ago, signed by both parents for leaving the country and entering the Shengen zone with attendent (both original and a copy / a verified copy by notary);
- both parents’ national passports copies (pages with their photos and registration stamps).
If a parents giving a notarized consent form to a grandmother or grandfather for accompanying a child should provide documents confirming relationship with a grandchild.
• if a minor travels without any attendant (only if a minor is 14 yars old or older):
- a notarized consent form without any attendant, issued no more than one year ago, signed by both parents with data of the
adult who will be responsible for the minor during stay in Schengen territory (both original documents and copies / a verified copy by notary);
- both parents’ national passports copies (pages with their photos and registration stamps);
Children under the age of 14 years old can travel only accompanied (by both parents, one of the parents or by third person).
• if a child travels with his/her legal guardian:
- a confirmation of an accommodation for the trip shared with the minor;
- round-trip tickets for the legal guardian and the child;
- a copy of legal guardian’s valid visa, foreign passport page containing personal data and his passport in original (in case of visa is applied for a minor only and legal guardian has already a valid Schengen visa);
- documents proving the termination of parental rights or the loss of parents (original documents and copies);
- court decision on the appointment of a guardian;
- consent form from the Council of guardians;
- financial guarantees by legal guardian;
• in case of deceased parent an original death certificate or in case of a certificate of termination of parental rights are provided under the following circumstances;
• form № 25 (a certificate taken in a regional Register Office) is required, when the father’s name is mentioned by the mother choice on child’s birth certificate;
• if a parent has gone missing or his whereabouts are not known, it is necessary to provide an appropriate police certificate.
