Minors
Submission of documents for minor citizens is possible only in the person of their lawful representative (one or both parents/the legal guardian).
To submit documents for a minor citizen (irrespective of them holding their own passport), in addition to the documents required the following must be provided:
- birth certificate of the minor (the original document and a copy or a notarized copy). If any of the parents or the minor has changed their name/surname after receiving the child’s birth certificate, they should provide documents proving it, such as marriage/divorce certificates or certificate from Bureau of Acts of Civil Status (the original document and a copy or a notarized copy);
- a filled-in application form signed by both parents on the last page of the application form along with the child’s photo;
- If parents have valid Schengen visas: copies of the valid visas, pages of the parents’ international passports with the photo and personal data and originals of their passports;
- both parents’/the legal guardian’s domestic passports (originals + copies of the main page and current registration);
- If applying for a visa for a minor accompanied by both parents in the trip, the following documents must be submitted:

• If applying for a multiple-entry visa, in addition to the abovementioned documents, the following must be submitted:
- reciprocal notary power of Attorney of travel consent from both parents (from father to mother, from mother to father) for the entire period of the requested visa (originals or notarized copies);

• If any of the parent leaves the Schengen countries earlier than their child or arrives there later, the following must be submitted:
- notary power of Attorney of travel consent from the other parent (original or a notarized copy);

• If the minor travels with their grandparent(s) or a third person, the following additional documents are required:
- return tickets of the accompanying persons and the child; For details…

- copies of domestic passports of accompanying persons (main page and current registration);
- copy of the valid Schengen visa, if applicable, the page of the accompanying persons’s international passport with the photo and personal data and original of the passport;
- notary power of Attorney of consent on travel with the accompanying person from both parents (originals or notarized copies);
- proof of relationship (if applicable);
• If the minor travels unaccompanied (older than 14 years old) the following additional documents are required:
- notary power of Attorney of consent on travel unaccompanied from both parents, indicating data of the adult who will be responsible for the minor during stay in Schengen territory (originals or notarized copies);
- copy of the identification document of the adult who will be responsible for the minor, as well as a copy of the document proving their legal residence in Schengen territory;
- confirmation of accommodation shared with the minor;
- if the column “father” in the child’s birth certificate is blank, it is required to submit the original and a notarized copy of the document;
- if the child’s father is indicated by the mother’s choice, form № 25 (a certificate taken in a regional Register Office) is required (the original and a notarized copy);
- in the event of death of a parent or termination of parental right, originals and notarized copies of the relevant document are required;
- if a parent has gone missing or their whereabouts is not known, it is necessary to provide the court decision (the original and a notarized copy).
